We are delighted to invite SCA Home Based Care members to a focused, interactive drop-in session with Jane Townson, CEO, and Paul Cooper, Senior Quality, Compliance and Innovation Specialist, from The Homecare Association. This is an important opportunity to share what the hospital discharge process in Surrey is really like for providers – what works well, what creates challenges, and what needs to change.

  • Have you experienced good practice – or serious challenges – during the hospital discharge process?
  • Have clients been encouraged to seek alternative support on discharge, rather than returning to their existing provider?
  • Have you found it difficult to secure placements or engage effectively through discharge routes?

We want to hear directly from providers about the realities of hospital discharge – the good, the bad and the ugly. Your insight will help build a clearer picture of what is happening on the ground and where improvements are needed. Where issues are identified, SCA will work with the Homecare Association to encourage changes in NHS and County Council processes, supporting a system that is fair, open, transparent and equitable for all social care providers. Stories and examples can be shared anonymously if preferred.
